General procedure for creating  and filling in  forms

The following steps are general procedures. Details are provided elsewhere in this Help file.

To create a form

1. Open a Word document or template.
2. Add or remove fields, as necessary, using square brackets to surround the field name. For example, [lastname].
3. Save the document as a regular Word document or as a template.

To fill in a form

1. Open the form, or Word document.
2. In Word 2007, click the FormsAssistant tab to display the FormsAssistant toolbar. In Word 2003, open the toolbar according to the steps in Restoring the toolbar, if necessary.
3. Click the FormsAssistant button, FA button,  to open the Main window.
4. Fill in the form by scrolling through the Fields list and entering the appropriate information in the corresponding Values column.
5. When you finish filling in the form, click Process on the FormsAssistant toolbar.
6. Save the form.
