| 1. | Open the form model and save it with a meaningful name. |
| 2. | Click the FormsAssistant icon on the Word FormsAssistant toolbar. |

| 3. | In the Main window Fields list, click a field's Value and either enter a new value or click an existing value from the Clips list. |

| 4. | Continue entering data into the Value column. |
| 5. | After entering all of the data, click Process on the FormsAssistant toolbar. FormsAssistant automatically closes when it finishes processing the file. |

| 6. | The form can be saved, printed, and treated like any other standard Word document. |
