Creating form models

You create form models using Microsoft Word. To create a form model, follow these steps:

1. In Microsoft Word, open an existing document that you want to use as a form, or create a new document.
2. Save the document with a meaningful name.
3. Enter text into the document just as you would any other Word document.
4. In the places where variable information is entered, insert field names using square brackets to identify the field. For example, [name], [address], and so forth.
5. If you want, you can use Word's highlighting tool to identify the fields in the text.
6. Save the document as a Word template or protect it with FormsAssistant, so you can reuse it.
