You create form models using Microsoft Word. To create a form model, follow these steps:
| 1. | In Microsoft Word, open an existing document that you want to use as a form, or create a new document. |
| 2. | Save the document with a meaningful name. |
| 3. | Enter text into the document just as you would any other Word document. |
| 4. | In the places where variable information is entered, insert field names using square brackets to identify the field. For example, [name], [address], and so forth. |
| 5. | If you want, you can use Word's highlighting tool to identify the fields in the text. |
